When you move, you should provide written notification of your new address to the new municipality to which you are moving, at least 5 days after moving.

You can notify of your change of address at your nearest Citizen Services Centre.

When the notice of your change of address has been submitted, you will receive a new National Health Service medical card within a month.

Change of doctor in relation to moving is free. Upon moving, you should choose a new doctor within a distance of 15 km of your new address. You may apply to retain the services of your current doctor.
